Anthony H Chavez

October 07, 1956 - June 04, 2010

of Lockport, passed away at Provena St. Joseph Medical Center, Friday, June 4, 2010. Age 53 years.

Survived by his wife of 16 years Rita C. Chavez (nee Smith), his brother Raymond Chavez, Jr., and his sister Sandra (Rigoberto) Martinez. Three sisters-in-law Avonell (Dennis) Hamblin, Jane Smith and Sheila (Chris) Spears. A brother-in-law W.P. (Brenda) Smith. Four uncles Joseph, Henry and Richard Chavez and Primo (Juanita) Gomez. Six aunts Carmen Perez, Essie Chavez, Gloria (William) Nussbaum, Carmen (Timothy) Alfaro, Theresa (Jose) Cantu and Mildred (Carl) Armbrust. His nieces and nephews Cristina and Daniel Martinez, Richard (Nicole) Smith, Michael (Brandi) Smith, Raymond (Erin) Smith, Jason Spears and Felicia Allender. His three precious chihuahua’s Martini, Rossi and Olive. Four great nieces and nephews and five God children. Also survived by numerous cousins.

Preceded in death by his parents Raymond (Jan. 2010) and Christine (1999) Chavez, an aunt Florence Bauer (May 2010) and two uncles Jack (Norma) Chavez and Michael Lopez (2001).

Anthony was a custodial engineer for Joliet Grade School District #86 for the past 21 years. He is a member of St. Bernard Catholic Church, Ingalls Park A.C., Belmont A.C., Eastside A.C., Loyal Order of Moose Lodge #1557 in Lockport, Lockport V.F.W. #5788 and the Lockport American Legion Post #18.
Anthony enjoyed spending time with his friends, playing euchre and was a die hard Chicago White Sox fan.
His family would like to extent a Thank-you to Drs. Ajmere, Lakhani and Jhawar for his care and treatment. Also a Thank-you to the Nursing Staff of Provena St. Joseph Medical Center West Wing for the wonderful care and concern provided during his stay.

Funeral Information

Funeral services will be held from the Carlson-Holmquist-Sayles Funeral Home, 2320 Black Rd., Tuesday, June 8 at 9:30 A.M. to St. Bernard Catholic Church at 10:00 A.M. for a Mass of Christian Burial. Interment Woodlawn Memorial Park. Memorial to a charity of your choice will be appreciated. Visitation will be held Monday from 2:00 – 8:00 P.M.
